Please note that we do not have any contracts.  We do however ask that you submit post-dated cheques for the year.  These cheques are returned if for some reason you decide that you are no longer interested in maintaining your membership with G.J.J.S.  Please allow 30 days notice for respect of others wishing to take your spot!

Emergency Services.............................................$50/month (**G.J.J.S. approved**)(4 hours/week instruction and open mat)

Adults (over 16)..................................................$50/month (3 hours/week instruction and TBA open mat)

Drop-In (Any class)..............................................$15/class

Private Instruction................................................$50/hour

The applicant hereby applies for membership & using the GJJS facility at Parksville location and agrees to observe all class rules and regulations established for maintaining order and protecting the members from injury and to respect the discipline of the instructors.

I confirm and agree as follows:

I am in proper physical condition to participate in Martial Art training and I suffer no medical condition which prevents me from participating or which might increase my risk of injury;

I understand that proper instruction in Martial Arts requires, that I learn mental and physical discipline, I therefore agree to observe all necessary rules and code of conduct for the purpose of maintaining and promoting order and to protect my classmates and myself from injury;

There are inherent dangers in the practice of Martial Arts or related Arts, and I may suffer injury as a result of practice and/ or instruction. I agree to accept all such risks of injury;

I release and discharge GJJS, including all Martial Art and related classes, all instructors, employees, administrators, agents and assign from any and all liability for damages or claims resulting from Martial Arts instruction at GJJS classes or any other course or activity offered by GJJS. This release is unrestricted and extends to all claims or damages whatsoever suffered by me including to my person, property, my future and whether of a physical, mental or monetary nature;

I agree that in the event of any damage or claim referred to paragraph 4, I will not make any claim or  take any proceedings which will subject GJJS or their founders or employees to any liability. Such proceedings or claims include those against third parties or through any government authority. I agree to indemnify and save harmless GJJS from and against all such liability;

The agreement from GJJS classes to provide instruction to me is sufficient consideration for my agreement to these terms and conditions;

 

In addition for Minor applicants

If the person who will receive instruction is a minor (under the age of 19), the undersigned confirms and agrees as follows;

that I am the parent or guardian of the minor and I am authorized to sign on such minor’s behalf;

I consent to the minor receiving instruction on the terms and conditions set out in this agreement;

adopt all representations and agreements set out herein on behalf of thew minor and I agree to indemnify and save harmless GJJS  from and against any and all liability for damages or claims however they may arise against GJJS and/or its instructors, employees, administrators and assign whether made by or on behalf of the minor, though government authority arising as a result of actions against third parties
